Below is the online tournament entry for the 2022 Men’s Masters Slow Pitch State Championships.

Please complete ALL fields below to complete the online tournament entry. In the description field, please include your team name and the class you are entering, along with your manager’s name, email, and phone number if they are not the person completing registration (there will be enough space) If you would instead prefer to register by mailing in a paper version of the entry form with a check (and avoid the $5.00 online credit card processing fee), you can download the paper application HERE.

All teams entering state tournament play should print and review the online advance tournament packet 30 days prior to their state tournament event. The packet can be printed HERE.
